What we do

HomeCarePathway is a searchable directory for finding home health, hospice, and personal care (PCS) agencies. Families and discharge planners can filter by location, insurance accepted, services offered, and Medicare star rating — then call, email, or visit an agency directly.

We built this because choosing in-home care is often rushed, stressful, and confusing — usually happening at a hospital bedside or during a short transition out of skilled nursing. Our goal is to put clear, comparable information in one place so you can make an informed choice without sifting through dozens of websites.

HomeCarePathway is completely free for patients, families, and discharge planners. There is no login required to search, no information you have to give up, and no subscription.

Where our data comes from

Listings are submitted and maintained by the agencies themselves. Each agency is responsible for the accuracy of its contact information, services, and insurance acceptance.

Medicare star ratings and CMS provider information are sourced from Medicare's Care Compare, the official public dataset published by the U.S. Centers for Medicare & Medicaid Services.
